Job Description:
A company is looking for a Palliative Care Nurse Navigator.
Key Responsibilities
Conduct comprehensive symptom assessments and facilitate
end-of-life care discussions
Engage with members to address clinical issues and manage
escalations efficiently
Collaborate with healthcare teams and support members with serious
illness throughout their care journey
Required Qualifications
Bachelor of Science Degree in Nursing
Compact unrestricted registered nurse (RN) license; New York,
Illinois, and/or California licensure preferred
At least 5 years of nursing experience, including 3 years in
outpatient palliative care or home hospice for oncology
patients
Certification as a Certified Hospice and Palliative Nurse (CHPN) or
oncology-related certifications preferred
Ability to work flexible hours, including occasional Saturdays and
up to 2 holidays per year
