Job Description:

A company is looking for a Care Manager (RN).
Key Responsibilities

Develops and facilitates complex care management activities, including personalized care plans for members
Coordinates care between members, families, and providers to ensure appropriate services are received
Monitors member progress and revises care plans as necessary to meet individual goals

Required Qualifications

Degree from an Accredited School of Nursing or a Bachelor's degree in Nursing
2 - 4 years of related experience
Registered Nurse (RN) State Licensure and/or Compact State Licensure required
Bilingual preferred
