      • Provide home-based physical therapy to patients who have lost mobility, strength and functionality due to an illness or injury
      • Work as part of a home health team which may include an RN, LPN, CNA, HHA, OT, COTA, PTA -and SLP, focused on the patient's plan of care and goals -
      • Observe movements, conduct assessments, document progress and modify care plan as needed
      • Assist patient with exercises and hands-on therapy to manage pain and increase mobility
      • Educate patients and family on their plan of care, therapy goals, home exercises, and self-careA few must-haves for Home Health Physical Therapists:
        • Graduate of an accredited Physical Therapist Program and active PT license in Colorado
        • Minimum of three -(3) -years of physical therapy experience, ideally -one (1) -year in home healthcare
        • CPR Certification (demonstration course required)
        • Knowledge of state and federal home health regulations
